Integrated care in action

A new suite of case studies shines a light on how health and care organisations and staff are working in new ways to improve health and wellbeing across England. Exploring seven themes, including health inequalities, mental health, and children and young people, the case studies illuminate how integrated care systems (ICSs) are supporting people to live healthier lives and receive the care and treatment they need in the right place, at the right time. ICSs are central to the delivery of the NHS Long Term Plan. Growing out of the current network of sustainability and transformation partnerships, ICSs will cover the whole of England by April 2021.
